My Chart Mobile App

If you would rather access your medical records from your phone, you can utilize the mobile app to view your medical records online.  They healthcare app is available on Apple iPhone and Android.  Once you download from iTunes or the Google store, simply enter your MyChart login credentials.  Note: you must have a current account or register for an account to access the app.

My Chart & Health Insurance Open Enrollment

Open enrollment period is here and the search for health insurance under the Affordable Care Act has begun. It is important to compare health insurance plans and benefits; finding doctors in network, affordable premiums, high vs low deductibles, and adequate coverage for your family. If you switch insurance providers, contact your healthcare provider or doctor to obtain your medical records. If your health insurance provider currently utilizes an EHR or EMR service then accessing your medical records with your new in network doctor should be a whole lot easier.

Register for a MyChart Account

—  To get started visit your healthcare provider’s My Chart patient portal and click “Sign Up Now.”
—  You will be asked to enter your activation code, as shown on your healthcare enrollment paperwork followed by a series of questions to confirm your identity, such as date of birth, postal code and your home phone number.
—  If you do not have a code, you will need to obtain a one-time activation code from your doctor’s office.
